So, a lonely bedridden patient requires palliative care. At the same time, he is, to put it mildly, not himself – accordingly, he cannot make decisions independently. The person is in the hospital, being fed through a tube, but the hospital cannot keep him indefinitely. At some point, his relatives (elderly people) learn that he is scheduled to be discharged in a couple of weeks and realize that the only thing to hope for now is that a social worker will visit him three times a week. And that’s it! The relatives are in a panic – no one knows what to do with him at home and how to care for him. Neither in their family nor in their circle is there a specialist trained to care for such patients! It is clear that the person needs to be placed in a specialized institution. But, first of all, there are queues for such institutions. Secondly, the application must specify a specific institution right away, as one can only apply for one spot in the queue. Thirdly, to get in line, a certificate from the family doctor and the person’s own application are required. However, due to the loss of physical and mental abilities, the person can no longer sign anything. A power of attorney that would allow someone to act on his behalf has not been arranged – he did not think about it in advance. With great difficulty, we managed to find out that in such cases, three hospital staff members can sign the application. However, we had to find this information out ourselves – no one in the hospital informed us about this, although there is a social worker there. Also, no one ever informed us that a temporary guardian could be appointed in such cases. Why not create a step-by-step guide that clearly outlines the algorithm of actions in such cases? With contact information, working hours, addresses, and other important information for all necessary services? After all, people are already under stress – and in social services (unfortunately, I know from experience), not all employees take the trouble to clearly and succinctly explain what needs to be done in a given difficult situation...
